Disability Support Pathways

We walk alongside families, providing guidance and support to navigate disability, health, and community service systems.

We assist with understanding support options, accessing assessments, co-ordinating referrals, and connecting with appropriate services.

Our goal is to reduce complexity, and confusion, build confidence, and create pathways to support clearer, more accessible, and easier navigation.

Early Support, Stronger Pathways, Brighter Futures

We are committed to strengthening early identification and support pathways for children who may be experiencing developmental delay, disability, or functional challenges.
 

Accessing the right support early can have a meaningful impact on a child's development, participation, and wellbeing. We work alongside families to identify concerns, understand their child's strengths and needs, and connect with appropriate services and supports. 
 

Our role is to help families navigate health, education, and disability systems, ensuring they have the information, guidance, and Cultural connections needed to make informed, decisions, plans and access timely support. 

Through a trauma informed, collaborative, community, and family-centred approach, we help create stronger pathways that support positive outcomes for children and their families whilst ensuring Cultural safety and guidance. 

Finding the Right Path

We make accessing disability support straightforward and culturally safe. Our pathways are designed to ensure you get the right care at the right time.

Eligibility

Support is available for Aboriginal and Torres Strait Islander community members and their families living with a disability or developmental delay across the Warrumbungle LGA. We help both NDIS participants and those seeking diagnosis or first-time support.

Referral Process

You don't need a formal referral to start yarning with us. Families can self-refer by walking in, calling our office, or being introduced by another health service. We then work with our GPs and Allied health specialists to build your medical evidence and support planning.

Connecting you with others who understand your journey through shared lived experience and yarn-up sessions with grass roots community members and Elders with disability.

Health Transport

We aim to provide safe, inclusive, reliable and free transport to ensure you can access the health services you need locally, across the Warrumbungle LGA and the WNSW LHD region.
